Dr. Joshua Zeichner, a board-certified dermatologist in New York City, explains, “Skin cells clump together in the pores, creating a blockage that traps oil beneath the surface. This can lead to an overgrowth of acne-causing bacteria, enlargement of the follicle, and subsequent inflammation.”

While some pimples or clogged pores emerge on the skin’s surface, others remain trapped underneath. Dr. Michelle Henry, a board-certified dermatologist, clarifies, “When bacteria and other substances attract immune cells to the blocked pores, inflammation occurs, resulting in the formation of a painful blind pimple.” The bacterial inflammation is the primary reason some pimples develop beneath the skin rather than on the surface.

Various individuals experience different types of acne, and the reasons behind why some develop blackheads or whiteheads while others experience deep, painful pimples remain unclear, as noted by Dr. Zeichner.

Why Deep Pimples Transform Into Hard Lumps Beneath the Skin

On occasion, deep cystic pimples can morph into hard lumps under the skin, lasting for months. Dr. Henry explains, “Deeper pimples, also known as nodular pimples, stem from blocked pores; however, nodular pimples occur when bacterial inflammation penetrates deeper into the skin, resulting in a more substantial and resilient formation. This explains why nodular and hard pimples are located deeper in the dermis and have a distinct lumpy texture.”
